Registration
You need the following when visiting us for the first time:
- Completed Intake Form and Membership Agreement.
- A copy of your current recommendation.
- Your CA Drivers License or CA ID.
*California State Medical Marijuana Identification Cards
are NOT accepted.
- You MUST bring your ID and your original recommendation
with you on your first visit.
- Your doctor must verify your recommendation. Depending
on your physician this can take several days.

Membership Qualifications
Peace in Medicine operates in strict compliance with California
Health and Safety Code 11362.5 (Prop 215) and 11362.7 (SB
420). You must be at least eighteen (18) years old, have a
state-issued photo identification, and a doctor’s recommendation
for medical cannabis to access dispensary services. Parents
or court-appointed legal guardians must act as caregivers
to obtain service on behalf of legally qualified patients
who are younger than eighteen (18) years old. We serve only
legally qualified patients and caregivers. Under no circumstances
whatsoever are services provided to persons who are not qualified
under California law. You must become a registered member
to obtain services. Any person who is not a member or qualified
patient, including minors, is not permitted within the dispensary
area of Peace in Medicine.
To become a member of Peace in Medicine:
Present the original copy of your doctor‘s recommendation
for use of medical cannabis along with your state-issued photo
ID to the staff at Peace in Medicine. We will make a copy
of your recommendation and return the original to you. Your
membership will be pending until a Peace in Medicine staff
member verifies the validity of your recommendation. Upon
verification, your membership and access to dispensary services
will become active. Your membership will be valid until the
date your recommendation expires. Please note that we must
verify your recommendation before dispensing medicine.
Designated Caregivers:
Peace in Medicine patient members may designate a primary
caregiver who is authorized to receive medicine on the patient’s
behalf. Please be advised that a primary caregiver is someone
who has “consistently provided for the housing, safety,
or welfare” of the patient (CA H&S Code 11362.5).
Under no circumstances may a primary caregiver obtain services
for himself or herself unless he or she is also a legally
qualified patient and registered as such with Peace in Medicine.
To designate a caregiver, a completed Primary Caregiver Designation
Form is required. Forms are available from member services
staff members.

Member’s Code of Conduct
Be a Good Neighbor:
As a member of the Peace in Medicine community, we ask you
to be especially mindful of our neighbors. We have worked
hard to establish a positive relationship with our neighbors,
the City of Sebastopol, and the police department. Please
be respectful of our neighbors’ rights, privacy and
property. Our license from the City of Sebastopol stipulates
that no one shall consume medicine within 500 feet of the
Gravenstein Station property.
We encourage our patients to help us keep our commitments
to our neighbors by not creating a nuisance or lingering in
the parking lot or sidewalk areas. Always be careful and courteous
when entering or exiting the parking lot. These simple precautions
will keep Peace in Medicine’s neighbor relationships
in good standing and help prevent any legal interference with
our operations. With your support we will be here to serve
you for years to come.
Peace in Medicine’s members must abide by the following,
and will be required to attest to this during the membership
process. Peace in Medicine reserves the right to terminate
membership for any violations of our Code of Conduct with
no warnings or second chances.
- You must be at least eighteen (18) years old and have
state-issued identification to access dispensary services.

to obtain service on behalf of legally qualified patients
who are younger than eighteen (18) years old.
- Any person who is not a member or qualified patient,
including minors, is not permitted within the dispensary
area of Peace in Medicine.
- All members are required to show state issued identification
and have their membership validated by Peace in Medicine
personnel prior to accessing dispensary services.
- No cell phones, pagers, cameras and any other recording
devices are allowed anywhere on Peace in Medicine’s
premises. Please leave them in your car or at home.
- No alcohol, illegal drugs, or weapons are allowed in
and around the Peace in Medicine’s premises.
- Members are limited to one dispensary visit per day.
Members agree to the limits on amounts of medical cannabis
according to state (CA H&S Code 11362.5 & 11362.7)
and local law.
- No consumption of cannabis is allowed within 500 feet
of Peace in Medicine, which includes all of the Gravenstein
Station and healing center premises.
- Never sell or otherwise distribute medication you obtain
at Peace in Medicine. You will be permanently excluded from
membership if you do so.
- Only qualified service animals are allowed inside the
premises, all other pets are not allowed.
- You should treat everyone on the premises with respect.
You may be asked to leave the premises if you use offensive
or abusive language or behavior.
- There is no loitering on the Gravenstein Station property.
- For your safety, place all medication and plants out
of sight before leaving the building.
- In the event of an emergency, please follow instructions
from the staff.
- Please be friendly to and support our neighbors with
your patronage!
